Fingers and thumb
Finger CR to the PIP joint, thumb CR to the 1st MCP; keep the digit parallel to the IR and take the thumb AP.
Patient and part
Seat the patient at the end of the table with the elbow flexed 90 degrees. Each finger gets a PA, a PA oblique and a lateral. For the PA, the palm is flat with the fingers slightly apart. For the oblique, rotate the hand 45 degrees laterally and rest the finger on a step sponge so it stays parallel to the IR. For the lateral, put the finger closest to the IR to cut OID: the index finger thumb side down, the 3rd through 5th fingers little finger side down. Tape or fold the other fingers out of the way. The thumb gets an AP, a PA oblique and a lateral. For the AP, rotate the hand internally until the back of the thumb lies flat on the IR; hold the other fingers back with the opposite hand. A PA thumb is possible but adds OID and blur. With the palm flat on the IR the thumb falls naturally into a 45 degree oblique. For the lateral, arch the hand with the fingers flexed until the thumb is in a true lateral.
